Primary Care Initiative Community Health Assessment
This report summarizes the findings of an assessment of barriers to obtaining health care in underserved communities in New York City, with a focus on community residents’ articulation of their experiences accessing primary health care.
The stimulus for the implementation of a community health assessment came from a Task Force on Primary Care convened by New York City Council Speaker Christine Quinn’s staff, assisted by the Commissioner on the Public Health’s System (an NYSHealth grantee).
