Care Focus
In collaboration with the New York Diabetes Coalition, the Hudson Center for Health Equity and Quality developed Care Focus, a system to manage the care of people with diabetes.
Care Focus is a searchable, reportable database of people with diabetes that enables medical practices to remind patients when they are due for examinations and tests. It helps doctors encourage patient self-management of diabetes through printable graphs and status reports.
