Rethinking Service Delivery for High-Cost Medicaid Patients
This United Hospital Fund Issue Brief presents an overview of the Medicaid High-Cost Care Initiative and highlights the program development work done by seven participating health care providers with grant support from the Fund.
The High-Cost Care Initiative was launched in 2005 to shape new practices and policies that would rationalize service use, improve health outcomes, and cut Medicaid costs. The Fund invited select New York City health care provider organizations to apply for research and planning grants to support the design of a new approach to serving high-cost Medicaid beneficiaries. This report provides background on the Initiative, approaches taken by the seven grantees, and their experiences. It also lays out the challenges to improving care for certain high-cost Medicaid beneficiaries, focusing on those with multiple and substantial needs who rely disproportionately on hospital inpatient services.
