Improving Diabetes Care in Midwest Community Health Centers With the Health Disparities Collaborative
The U.S. Bureau of Primary Health Care’s Diabetes Health Disparities Collaborative sought to reduce health disparities and improve diabetes care in 19 Midwestern health centers.
In 1998, a study was conducted to summarize the results of a diabetes quality improvement initiative in Community Health Centers. This document informs the work of the NYSHealth Diabetes Campaign as it works with community health centers throughough the state to improve diabetes management.
