Seven County Diabetes Network

Seven County Diabetes Network is a coalition that targets individuals at risk for, or already diagnosed with, diabetes throughout Warren, Washington, Saratoga, Essex, Hamilton, Franklin, and Clinton counties in New York. They work collaboratively with health care facilities, public health departments, and community organizations to increase public awareness of risks, prevention strategies, management issues, and available local resources for people with diabetes or at risk of developing diabetes.
