Improving Chronic Illness Care
Improving Chronic Illness Care, a national program of the Robert Wood Johnson Foundation, is dedicated to the idea that the U.S. health care system can do a better job of caring for Americans with chronic illnesses, such as diabetes, and helping them lead healthier lives.
Likewise, providers who care for chronically ill patients can be better supported with guidelines, specialty expertise, and information systems. Better care delivery, the sponsors believe, also will reduce overall health care costs.
