Health Disparities Collaboratives is a nationwide initiative to improve care for people with chronic conditions, including diabetes.
This initiative of the Health Resources and Services Administration, Bureau of Primary Health Care, and the Institute for Healthcare Improvement, seeks to achieve excellence in care through building strategic partnerships; transforming clinical practice through models of care, improvement activities, and learning; developing infrastructure, and documenting health outcomes. The program’s Web site includes training manuals, tools, resources, and a calendar of events.
