Many patients who are diagnosed with diabetes lack adequate treatment and do not know how to manage a chronic disease. Further, additional health problems that may develop can create hindrances to receiving consistent and effective care. In an effort to help, this project continues to advance the Diabetes Campaign goal of improving health outcomes for patients with diabetes. The Community Health Care Association of New York State (CHCANYS) dedicates staff time and resources to work with 20 sites and assist at least 75 additional providers with achieving recognition under the National Committee on Quality Assurance (NCQA) Diabetes Physician Recognition Program or the Bridges to Excellence (BTE) Diabetes Care Link.

The NYSHealth Diabetes Campaign selected CHCANYS as its first clinical partner to participate in a five-year, $35 million campaign, with the goal of reversing the diabetes epidemic in New York State by focusing on improving clinical care, mobilizing communities, and promoting policy.

CHCANYS has been one of the most active clinical partners working to advance the Campaign goal of 3,000 primary care physicians achieving the best outcomes for patients with diabetes, as demonstrated by attaining recognition by the NCQA or the BTE diabetes programs. As of July 2010, CHCANYS has worked with 120 providers, 56 of whom have achieved recognition by NCQA. Improving diabetes outcomes for the population served by CHCANYS’ member sites can significantly reduce diabetes complications and hospitalizations in New York State.
