Foundation Strategies

In 2008, NYSHealth launched a five-year, $35 million campaign to reverse the diabetes epidemic in New York State. The vision for the NYSHealth Diabetes Campaign is to transform care and reduce the human and financial toll associated with diabetes. The Campaign focuses on three key strategies:

Improving Clinical Care: We seek to build a critical mass of health care providers in New York State that achieve national recognition for excellent diabetes care.

Mobilizing Communities:
Working with faith-based organizations and other community-based organizations like YMCAs, we aim to improve diabetes prevention, screening, and management.

Promoting Policy:
NYSHealth established a Diabetes Policy Center to conduct and fund policy analysis and advocacy to support private and public payment policies that sustain a comprehensive, coordinated care system for patients with diabetes.

Open Request for Proposals

The Meeting the Mark: Achieving Excellence in Diabetes Care request for proposals (RFP) invites organizations across New York State to apply for funds to improve the quality of care for people with diabetes.
