Organizations Working to Improve Diabetes Outcomes

Throughout the State there exist programs that impact individuals, communities, environments, and promote policy change to reduce the risk of diabetes and obesity. These activities may include school-based health programs, work-based initiatives, or community-based programs such as building of bike and walking paths, walking groups, fitness programs, farmer’s markets and community gardens. These activities coupled with policy initiatives work to decrease the incidence of obesity and improve physical and mental health.

National Certification Board for Diabetes Educators

The National Certification Board of Diabetes Educators conducts certification activities and awards the credential of “Certified Diabetes Educator” (CDE). The CDE credential demonstrates that a health care professional possesses distinct and specialized knowledge, thereby promoting quality care for persons with diabetes.

American Association of Diabetes Educators

The American Association of Diabetes Educators (AADE) provides members with tools, training, and support necessary to help patients change their behavior and accomplish their diabetes self-management goals.

American Diabetes Association

American Diabetes Association conducts programs in all 50 states. The Association’s mission is to prevent and cure diabetes, and to improve the lives of all people affected by it. To fulfill this mission, the Association funds research, publishes scientific findings, and provides information and other services to people with diabetes, their families, health professionals, and the public. The Association’s Web site promotes better diabetes self-management through healthy food choices and meal planning, exercise, and weight loss.

Bridges to Excellence

Bridges to Excellence is a multi-stakeholder program designed to advance the “pay for performance” concept throughout the U.S. health care system.

Improving Chronic Illness Care

Improving Chronic Illness Care, a national program of the Robert Wood Johnson Foundation, is dedicated to the idea that the U.S. health care system can do a better job of caring for Americans with chronic illnesses, such as diabetes, and helping them lead healthier lives.

North Eastern New York Diabetes Educators

North Eastern New York Diabetes Educators, a chapter of the American Association of Diabetes Educators, provides educational opportunities for the professional growth of diabetes educators, promotes and aids quality diabetes education for the person with diabetes, and fosters collaboration among individuals and organizations involved in diabetes education.

Seven County Diabetes Network

Seven County Diabetes Network is a coalition that targets individuals at risk for, or already diagnosed with, diabetes throughout Warren, Washington, Saratoga, Essex, Hamilton, Franklin, and Clinton counties in New York. They work collaboratively with health care facilities, public health departments, and community organizations to increase public awareness of risks, prevention strategies, management issues, and available local resources for people with diabetes or at risk of developing diabetes.

The Diabetes Quality Improvement Project

The Diabetes Quality Improvement Project is a coalition of public and private entities representing generalist physicians; specialists in diabetes care and education; experts in health services research, epidemiology, and statistics; health plan administration; and others. The group has established a set of diabetes-specific performance and outcome measures, based on scientific evidence, that are user-friendly to payers and consumers, allow for fair comparisons of health care plans, and stimulate quality improvement.

Community Coalitions for Diabetes Prevention

Community Coalitions for Diabetes Prevention comprise regional partnerships working to prevent diabetes and its complications in 15 locales across the State. Each coalition is made up of partners from community-based organizations dedicated to health improvement.

New York Diabetes Coalition

New York Diabetes Coalition is a collaboration of medical societies, health plans, health care-focused agencies, and community organizations is dedicated to improving the health of people who live with diabetes.

New York State Diabetes Centers of Excellence

New York State Diabetes Centers of Excellence are hospital-based diabetes specialty centers. These five centers are working to turn research into effective diabetes prevention programs in communities. These centers are leaders in research, treatment, and education, offering high-quality care and cost-effective services to patients.

New York State Diabetes Prevention and Control Program

New York State Diabetes Prevention and Control Program supports partners throughout the state to improve diabetes care.

Cornell University Cooperative Extension-New York City

Cornell University Cooperative Extension-New York City (CUCE-NYC) responds to emerging issues through special initiatives that improve the nutrition and health status of residents in targeted communities.

City University of New York (CUNY)

The City University of New York (CUNY) has launched the CUNY Campaign Against Diabetes to strengthen CUNY's capacity to prevent Type 2 diabetes among students, faculty, staff, and their respective family members. The Campaign mobilizes CUNY’s teaching, research, and service capacities to improve the management and prevention of diabetes in the CUNY community.
