Sugar-Sweetened Beverages and Risk of Metabolic Syndrome and Type 2 Diabetes

July 2011

This November 2010 Diabetes Care article analyzes the intake of sugar-sweetened beverages and the risk of metabolic syndrome and Type 2 diabetes. Regular consumption of sugar-sweetened beverages has been associated with weight gain and the risk of obesity, but their role in the development of chronic metabolic diseases had not been previously been reviewed.

Projection of the Year 2050 Burden of Diabetes in the U.S. Adult Population: Dynamic Modeling of Incidence, Mortality, and Prediabetes Prevalence

July 2011

This August 2010 Population Health Metrics article projects that annual new cases of diagnosed diabetes incidence will increase from about 8 cases per 1,000 in 2008 to about 15 in 2050. As the rate of diabetes incidence continues to rise, researchers project that up to 33% of the U.S. adult population could have the disease by 2050, up from 14% in 2010.

Scheduled and Unscheduled Hospital Readmissions Among Patients With Diabetes

This October 2010 American Journal of Managed Care article looks at the rates of scheduled and unscheduled readmissions among midlife and older patients with diabetes and examines the socioeconomic and clinical factors associated with these rates.

This issue from The Lancet, in collaboration with the American Diabetes Association, is dedicated to the global diabetes epidemic. Today, diabetes represents a much more complex problem than just insulin control and this issue examines the creation and implementation of global and local health policies to address the current pandemic of diabetes.

This annual report from the Trust for America’s Health and the Robert Wood Johnson Foundation tracks obesity trends state by state and finds that the adult obesity rate has increased in 16 states in the past year and did not decrease in any.

Guidelines for Adult Diabetes Care

Developed by the New York Diabetes Coalition, in collaboration with the New York State Department of Health’s Diabetes Prevention & Control Program, these guidelines offer minimum recommendations on areas such as A1C levels, blood pressure, cholesterol, and smoking status. The guidelines are based on the American Diabetes Association Clinical Practice Recommendations and are reviewed yearly.

Standards for Patient-Centered Medical Home 2011

This new set of standards from the National Committee for Quality Assurance is an innovative program for primary care. The standards provide clear and specific criteria to help primary care providers organize their practices into more patient-centered care emphasizing strong coordination and integration of care; provide comprehensive services delivered as part of a team-based approach to care; and foster a robust relationship between patients, their families, and the provider.

The New York State Community Health Worker Initiative

This NYSHealth-funded report provides background on the state of the Community Health Worker (CHW) field, the emerging evidence supporting use of CHWs, and the opportunities to advance the field in New York. CHWs are frontline public health workers who are trusted members of the community they serve. They function as liaisons between communities and health and social services, facilitating access to services as well as building individual and community capacity.

Community Health Workers: A Critical Link for Improving Health Outcomes and Promoting Cost-effective Care in the Era of Health Reform

This working draft of an NYSHealth white paper explores how Community Health Workers (CHWs) are being given a larger role in primary care as a result of the Patient Protection and Affordable Care Act (PPACA). As emerging members of effective care management teams, CHWs have been shown to play a critical role in addressing the drivers of health care costs and improving quality of care for patients.

Defy Diabetes Presents at Annual AADE Meeting in Atlanta

Kelly Brownell, Director of the Yale Rudd Center, and Thomas Frieden, Health Commissioner of New York City Department of Health, composed a report highlighting that taxes on soda have significant potential for improving health. This report released by the New England Journal of Medicine provides perspective on a controversial public health measure to prevent diet-related diseases.

New York State Strategic Plan for the Prevention & Control of Diabetes

The New York State Department of Health’s Strategic Plan for the Prevention and Control of Diabetes was created in conjunction with the New York State Diabetes Task Force, an advisory body of diabetes experts.

Practice Facilitators and Practice-based Research Networks

This article provides examples of how the practice facilitator model was implemented in four practice-based research networks in the United States. NYSHealth is supporting grantees that use this model to improve quality of care for patients with co-occurring conditions for mental health and diabetes.
