Cost Effectiveness for Diabetes Care

Too often, patients with chronic diseases like diabetes are forced to cope with poor quality care or forego it entirely because of a lack of providers throughout the State. The result is poorer health and more frequent hospitalizations when conditions become serious, which in turn drives up costs.
However, the high costs associated with the care for chronic diseases like diabetes can be reduced through preventive remedies, including pre-screening and tracking the health of high-risk patients. NYSHealth is proud to support the efforts of four grantee organizations that are working at all levels across the State to reduce the cost of care for those with chronic conditions and improve its effectiveness.
