Medical Home
“Medical Homes” are critical to ensuring families receive excellent primary and preventive care services, particularly for children. Unfortunately a decade-long decline in primary care providers across the State has severely reduced the number of primary care doctors and health centers, leaving many communities without access to the health services they need.
Without a medical home, many families are forced to use emergency rooms to treat common illnesses, and chronic conditions too often do not receive the consistent treatment needed to prevent complications. Addressing this issue requires a mixture of on-the-ground capacity building and State-level policy work, and NYSHealth currently supports two grantees doing just that.
